- Mondays, Fridays, Saturdays: 9:00 AM – 12:00 PM / 2:00 PM – 5:00 PM
- Up to 3 people per 30-minute slot
Eligible Individuals:
- From 6 months old and above
- For children under 13 years old, it is recommended to receive two doses at an interval of 2–4 weeks.
Vaccination Cost:
- ¥4,000 (per person, per shot, tax included)
Additional Information:
- Residents of Kutchan who receive subsidies need to complete a specific medical questionnaire. Please pick it up beforehand or fill it out on the day of your vaccination.
- If you receive a subsidy from a municipality other than Kutchan, you must present a vaccination request form issued by your municipality. Please contact your municipality in advance before making an appointment.
- For companies or organizations wishing to make a group payment, a dedicated reservation menu will be created. Please consult our clinic in advance.
- For groups of 10 or more people, we also offer on-site vaccinations at your company or organization. If interested, please contact our clinic.
